For in-home, non-medical personal care you can trust for yourself or your loved one, choose Nealen Personal Care, Inc. Attendant Care Program

Through our Attendant Care Program, we provide in-home, non-medical personal care to help the elderly who are physically disabled yet mentally alert, maintaining their independence so they can stay in the comfort of their homes. These services are offered through the Area Agency on Aging, to clients who are ages 60+ and were previously served by the Department of Public Welfare’s Attendant Care Program.

Services Provided

Helping client get into and out of bed, wheelchair or motor vehicle

Bathing and bathroom assistance

Personal hygiene, dressing and grooming assistance

Meal preparation and cleanup

Light housework, including laundry

Medication

Errands

Companionship

Range of motion exercises

Aging Waiver Program

The Pennsylvania Department of Aging Waiver (PDA Waiver) is a long-term program designed to help elderly Pennsylvania residents live independently in the comfort of their own homes. Care management and coordination is provided through the Area Agency on Aging.

Eligibility

You must be a Pennsylvania resident, at least 60 years of age, and meet all of the following criteria

Would benefit from nursing facility care

Can be serviced in the community by health and social services available

Meet the financial requirements, as determined by your local county assistance office

Agree to the requirements and responsibilities of the program


Family Caregiver Support Program

The Family Caregiver Support Program strives to support the in-home care provided to older family members. Eligibility for this program is based on income and functional needs. For more information and to apply for the program, contact your Area Agency on Aging at (814) 539-5595.

Independence Waiver Program Eligibility

Pennsylvania residents ages 18 to 59 with a severe physical disability that requires a nursing facility level of care. The disability must cause substantial functional limitations in three or more of the following major life activities: mobility, communication, self-care, self-direction, capacity for independent living, and learning.


OBRA Waiver Program Eligibility

Pennsylvania residents ages 18 or older with a severe developmental physical disability requiring an Intermediate Care facility/Other Related Conditions level of care. The disability must result in substantial functional limitations in three or more of the following major life activities: mobility, communication, self-care, self-direction, capacity for independent living, and learning.


Attendant Care Program Eligibility

Pennsylvania residents ages 18 through 59 who are mentally alert and have physical disabilities.
The purpose of the PA Attendant Care Program is to enable individuals with physical disabilities to perform daily living activities.


Attendant Care Act 150 Eligibility

Be a U.S. citizen and Pennsylvania resident

Be between 18 and 59 years old

Be mentally alert and capable of:
*Managing one’s own financial affairs
*Managing one’s own legal affairs

Experience any medically determinable physical impairment that can be expected to last for a continuous period of no less than 12 months or that may result in death.

Due to physical impairment, require assistance to complete functions of daily living, self-care and mobility.


The Pennsylvania Independent Enrollment Broker

The Office of Long-Term Living launched the PA Independent Enrollment Broker, effective December 1, 2010. The Independent Enrollment Broker is a contracted statewide entity to facilitate and streamline the eligibility/enrollment process for applicants seeking services for several Pennsylvania waivers/programs.

The Independent Enrollment Broker provides enrollment services for applicants with physical disabilities who are 18-59 years of age applying for Attendant Care, COMMCARE, Independence, OBRA and the 0192 (AIDS) Waivers and the Act 150 Attendant Care Program. Area Agencies on Aging (AAA) provide eligibility/enrollment services for applicants over age 60.

The PA independent Enrollment Broker will work in close collaboration with service coordination providers and key stakeholders to respond to needs, address issues, and ensure participants receive prompt, high-quality service.

  • Q: I need in-home care for my loved one and I feel so overwhelmed. Where do I begin?

    A: We hear this statement often. It’s easy to become confused and overwhelmed when weighing your
    options for in-home care.

    We suggest starting with your county Area Agency on Aging (AAA) to see if you qualify for one of their programs: Options or the Family Caregiver Support. The AAA will conduct a free, in-home evaluation and will tell you if you are eligible for any of their programs. If you are eligible, they will give you a list of Home Care Agencies in your area that can perform these services, and Nealen Personal Care is on this list. They can also give you information on Meals on Wheels and Personal Emergency Response systems. The phone number for the Cambria County Area Agency on Aging is (814) 539-5595.

  • Q: How do I start my family member on a waiver program?

    A: If you or your loved one is 60 or older, you may qualify for the Aging Waiver program. Currently, Maximus, the Pennsylvania Independent Enrollment Broker, can assist you with this. Contact them at 1-877-550-4227.

    It may take a while to get these services into place, especially waiver services, so please be patient. If there is a waiting list for the services you want, ask them to add your name to the list.

  • Q: Does Nealen Personal Care offer private pay services?

    A: Yes, we also provide private pay services. Please contact our office for our current rates.

  • Q: Do you offer overnight/weekend coverage?

    A: Yes, if staffing is available.

  • Q: For private pay, is there a minimum number of hours you require?

    A: We prefer to work a minimum two-hour shift. We do, however, work one-hour shifts. Call for our rates.

  • Q: What services do you perform?

    A: Nealen Personal Care, Inc. is a non-medical personal care agency. We assist with the Activities of Daily Living (ADLs), such as bathing, dressing, bathroom assist, laundry, errands, light housekeeping, dishes, and med reminders.
